REFERRING PATIENTS TO AVELLA INJURY AND PAIN
Pain management can be very time consuming for primary care providers, as patients may try multiple treatment methods to varying degrees of success. If you have a patient who requires further chronic pain management or injury rehabilitation, you can refer them to Burlington’s Avella Injury and Pain Management.
Our goal is to develop treatment plans that are sustainable and effective, reducing pain and improving function. We prefer non-opioid treatment whenever possible, and we are experienced with medical cannabis.
Many of our treatments are covered by OHIP or third-party health insurance providers.

Patients at Avella Injury and Pain Management require a referral from their primary healthcare provider. Please download our referral form below, fill out, and submit it via fax to 289-714-2516. If you have a particular question about your patient’s pain management which needs to be addressed promptly, please contact us today.
