HOW TO BECOME A PATIENT

For patients interested in chronic pain management, before coming to see us, you will require a referral from your primary healthcare provider. Your primary healthcare provider will need to complete our patient referral form, which can be downloaded below.

For patients aged 25 years and older, interested in medical cannabis, please contact us.

For patients interested in physiotherapy or in massage therapy, there is no need for a referral, kindly call us at 905-315-7007.
