What to Expect
YOUR FIRST VISIT TO AVELLA INJURY AND PAIN
Welcome to Avella Injury and Pain Management in Burlington, Oakville, and Milton. We look forward to meeting with you and discussing your goals and personalized needs. Once you have obtained a referral from your primary healthcare provider, here are some things to keep in mind about what to expect and how to prepare for your appointment.
INSURANCE
Please bring your valid OHIP card to every appointment, as well as any third-party insurance information, such as WSIB, accident benefit and extended health plans.
NERVE BLOCK PATIENTS
For our nerve block patients, please do not drive yourself home. We suggest you bring an accompanying driver or arrange for a ride.
CLOTHING
We recommend that you wear comfortable clothing that can be removed easily.
MEDICATIONS AND ASSISTIVE DEVICES
Please bring a list of recent and past medications printed from your pharmacy. If you are unable to do this, please bring the medications with you. Additionally, please bring any assistive devices you use, including eyeglasses, hearing aids and more.
